Appendicitis in an infant – diagnosis with his mom
A 1,5 months’ infant with appendicitis was operated at the RSPC for Pediatric Surgery. This is the first registered case in so young infant for the last 50 years.
“Appendicitis as a spontaneous pathology in young infants is a very rare phenomenon,” says Alexander Svirsky, head of the Department of Pediatric Surgery at the Center. “World literature describes less than a thousand such cases. Most often, the appendix is inflamed against the complications of necrotic enterocolitis and intestine development defects. According to statistics, these are usually destructive forms complicated by peritonitis.”

The goal is omphalocele. 10 days for treatment
The RSPC for Pediatric Surgery has improved the technique of eliminating embryonic hernias (omphalocele) of large size, and has carried out 3 operations with its help.
In Belarus, averagely one or two children per year are born with an omphalocele. The fault develops in the intrauterine period: the anterior abdominal wall does not close, the intestine, part of the liver and other internal organs protrude from the abdominal cavity, the hernia sac is covered with embryonic membranes. The abdominal cavity of the newborn is very small. This is the whole difficulty of treatment: how to restore the correct anatomical structure of the cavity, if there is not enough space? If intra-abdominal pressure will greatly increase – the internal organs will be squashed.

A big heart for a small man
Samir has already spent his first month in the intensive care unit of the RSPC for Pediatric Surgery. The most severe congenital malformation, a syndrome of a hypoplasia of the left divisions of the heart, doesn’t allow him to be near his Mom and Dad. Cardiac surgeons have only gone half way: two operations have been performed. There are two more ahead.
The parents of the baby new about the diagnosis at the 12th week of the pregnancy. The specialists from the RSPC Mother and Child joined in immediately. Samir was born in the Center, then the boy was urgently transferred to cardiac surgery.
“With the diagnosis like this, you need to operate during the first month, because in fact the child has only half of the heart,” says Dmitry Leskovsky, head of the Department of Anesthesiology and Reanimation, Intensive Care, Extracorporeal Circulation of the RSPC for Pediatric Surgery (on the photo). “The boy was born 2500 g, he received support before the beginning of surgical treatment.”

A vascular puzzle solved
For the smiling Maxim and his mother Elena, Valentine’s Day was truly a holiday: they were discharged from the RSPC for Pediatric Surgery of the Republic of Belarus. The child was cheerful and happy, as if he had never been ill. Only the bandage and slight nausea reminded of the operation.
The doctors will remember a five-year-old boy for a long time: one meets the patient like this only once in a lifetime. Correction of diaphragmatic hernia and abnormal vascular arterial network in the region of the left lung, esophageal opening of the diaphragm, stomach and liver became a real puzzle. The pediatricians and radiologists started to solve the problem, the surgeons finished.

Operation ‘Life’
The specialists of the RSPC for Pediatric Surgery operated a patient with a rare neuromuscular pathology of the esophagus – achalasia. A schoolgirl had a laparoscopic esophagocardiomyotomy.
“Achalasia is not widespread, especially in children: in our Center we fix such diagnosis approximately once in every 2 years. However, in 2015, we observed 2 such patients,” explained Alexandr Makhlin, the head of the operating team, deputy director of the RSPC. “A spasm of cardia happens, and as a result, obstruction develops. Disturbance of swallowing, unpleasant sensation of a coma in the throat, nausea aggravate for several years. Food is only semi-liquid, washed down with water.”

Team approach to Ehlers – Danlo
A 13-year-old female patient underwent a one-stage operation. A mitral valve was prosthetically inspected and the funnel-like deformation of the thorax was eliminated.
A week after the intervention, Vika sees the first changes: there is no cosmetic defect anymore, it’s easier to breathe and to move. The confidence of the girl and her parents is supplemented by the support of the attending physician, a cardiac surgeon Mikhail Shved (on the photo), and all the staff of the Cardiac Surgical Department No. 2 of the RSPC for Pediatric Surgery. They have known each other for more than 7 years.

Ilyusha and his good luck
Ilyusha, a month and a half, is finally at home. His Mom bursts the tears with difficulty: I can’t take my eyes off the son. I want to see him all the time, to be near him.
The child is wanted. Pregnancy was easy. Marina, the doctor herself, followed all the instructions of obstetrician-gynecologists, visited ultrasound procedure regularly. Terrible news was learned after the delivery in the intensive care unit (due to the weakness of labor, the doctors had to urgently perform cesarean section): Your son has a complex pathology: congenital diaphragmatic hernia, surgery is required.
